Primary Care Network: Social Prescribing
Many things can affect your health. A lot of people visit their GP because of feeling lonely or isolated. Or they might be stressed due to lack of work, employment or money problems. Your GP and the practice staff are able to refer you to Ashford Medical Partnership's Social Prescribing Service. The service is coordinated by a Community Link Worker who has the time to listen to you and connect you to people and activities that may help you to feel better.
The Community Link Worker will work with you to:-
- discuss the problems you are facing
- explore what matters to you
- develop a support plan with your goals
- find local activities and services
- help you access them
